The lift came from Harbor Freight. I did not get it while on sale, so I paid the full price, $349.
Another $20 will have it delivered right to your door.
To put the rear wheel elevated, I strap the front down and use a jack under the frame to lift the rear - tire changing, chain lube, etc;. You might notice the rear of the table is removable so tire changing is a whole lot easier.
For lifting, I've made some adaptors from wood to fit under the frame and not cause any damage. Scrollsaw and some plywood and laminate them together.
Like I said, I really like working on the bike at MY level, not it's level.
