Glad it is all worked out for you. It is very unusual that the clutch actuating arm had been moved on the splines. I wonder what cause the PO to do that?
Whatever, your clutch has been receiving some abuse and accelerated wear and so I would recommend, next time that you do an oil change, that you grab a new clutch cover gasket as well and do the other part of the clutch adjustment.
